Thank you for taking time to fill out this form.  Your Professor, Professor Havranek, is very interested in improving her craft.  While the end of semester evaluations are a source of information, they are often not the best source and do not ask pointed questions about specific aspects of the course that help, hinder, or could be improved upon.  That is why she has asked me to have you fill out this form.  You will notice that the form asks you to answer three questions and to provide three ideas/suggestions for each question.  Once students have completed the form, I will tabulate the information and provide a detailed summary for Professor Havranek.  Let me also assure you that the information you provide is completely anonymous.  The answers you provide come to me in the form of an email message but they do not indicate who sent them (it simply says that they came from the server).  So please take a few minutes to consider seriously your responses.  Not only will you provide valuable feedback to Professor Havranek as she seeks to become the best teacher she can be, you will also be doing a good service to the future students who will be taking this course.
